General movements at term and fidgety age in relation to neurological outcome in a group of twins

      Objective: Twins are at higher risk of an unfavourable neurological outcome, which can be predicted by assessment of the quality of general movements (GMs). There are contradictory results regarding the development of children born after medically assisted conception. The aim of this study is to analyze the quality of GMs and neurological outcome regarding the gestational age (GA) and mode of conception in a group of twins. Methods: We performed a retrospective analysis of medical documentation of 161 children: 62 born after assisted (A) and 99 after spontaneous (S) conception. Their GA was 24 to 38 weeks (M 34.8; SD 2.8), birth weight 670 g to 3820 g (M 2237; SD 543). The results of the assessment of GMs in both groups were analyzed in relation to neurological outcome at the age of two years. Results: In the group A 65% of children had normal GMs at term and 66% at fidgety age. In the group S normal GMs at term age were present in 64% and in 66% of children at fidgety age. Normal neurological outcome was present in 81% from A and 88% from S group. There was no statistically significant difference between the two groups in the quality of GMs, duration of gestation and neurological outcome. The groups differed statistically significant only regarding speech and language development (p<0.05). Conclusion: The mode of conception does not influence the duration of gestation, quality of GMs and neurological outcome. The influence on language should be further analyzed.
